CARE DIRECTIONS CASE MANAGER, SPANISH BILINGUAL

Under supervision, the individual provides assessment, screening, housing information, assistance with applications, coordination of referrals, follow-up and assistance with housing opportunities for individuals living with HIV/AIDS and their families. Requirements include: Bachelor’s degree and two years experience in direct human/social services; good writing/oral presentation skills; experience working with diverse populations and sensitivity to issues associated with HIV/AIDS, chemical dependency, mental health and sexuality; experience conducting interviews and assessments; previous experience with HUD and housing programs/providers desirable; reliable transportation with a valid Arizona driver’s license and insurance required.
