Through Los Ancianos, AmeriCorps members and volunteers are trained and assigned to the Agency or one of its thirteen partner organizations to provide new or enhanced services primarily in clients’ homes. Activities may include friendly visits and phone contact, light housekeeping, grocery shopping, meal preparation, and coordination of needed transportation. Through a collaboration with local hospitals, members and volunteers also help recently discharged patients avoid re-admission by providing services such as medication reconciliation, prescription pick-up, and ensuring that follow-up doctor appointments have been scheduled.
Other members and volunteers serve at newly created “wellness centers” at 10 senior centers in Maricopa County that serve primarily rural, minority, and/or low-income populations. Members and volunteers are trained to facilitate health promotion and education activities that include a variety of health screenings and implementation of evidence-based programs, including Matter of Balance, Chronic Disease Self-Management, and a Fitness Program.
Positions Available: Care Coordinator- Provide supportive services to individuals recently released from the hospital. Services include friendly visiting, weekly phone calls, picking up prescriptions, educating clients on how to complete their personal health records, scheduling & escorting clients to follow-up doctors’ appointments, running light errands, light housekeeping.
This position will be assigned to the Healing@Home Pogram. Healing@Home is a community based Care Transitions Program that targets Medicare fee for service (FFS) beneficiaries who are at high risk for hospital readmission.
