Healing@Home is a special program for people with Medicare who may be at risk of returning to the hospital. It is a pilot program administered by the Area Agency on Aging, Region One and funded by the Centers for Medicare and Medicaid Services (CMS).
Only individuals with Medicare Fee-for-Service (FFS) who are not enrolled in a Medicare Advantage plan are eligible to participate in the Healing@Home program.
You must be determined to be in need of Healing@Home services by the hospital discharge team and qualified by the Healing@Home Transition Coach. Your written consent is required for participation.
Healing@Home is only available at participating hospitals that have partnered with the Area Agency on Aging for the purpose of piloting the Healing@Home program.
Services you may receive as a qualified participant of the Healing@Home Program:
Home visits
Assistance with scheduling medical appointments
Education on diagnosis
Medication reconciliation
Telephone reassurance
Home safety check
Referral to home and community based services
