NOTICE OF REQUEST FOR PROPOSALS
The Area Agency on Aging, Region One, Incorporated, hereby solicits sealed proposals from qualified offerors to deliver services to elderly and disabled adults in Maricopa County, Arizona.
Contracts awarded pursuant to this Request for Proposals (RFP) will be for a term of twelve (12) months with the option to renew as follows: for a maximum of one (1) additional term for adult day health care and home care and for three (3) additional terms for all other services. Contracts awarded pursuant to this RFP are scheduled to begin July 1, 2008.
Proposals will be accepted for the following services to be provided in all Planning and Service
Areas (PSAs):
Adult Day Health Care Home Delivered Meals
Case Management Information & Referral
Congregate Meals Legal Services
Family Caregiver: Caregiver Training: Multipurpose Center Operations
Peer Counseling: and Supplemental Svcs Public Health Education
Home Care: Home Nursing; Transportation
Home Health Aid; Personal Care; Volunteer Management
Homemaker/Chore; and Respite
Copies of the official RFP document will be available after 12:00 noon on Wednesday, January 9, 2008 at the Area Agency on Aging office located at 1366 East Thomas Road, Suite 108, Phoenix, Arizona. Interested offerors are encouraged to obtain a copy of the document prior to the proposers’ conference. Proposers will be given a CD disc of the Word and Excel files for the RFP Submittal sections only.
Proposers’ conferences will be held at the Area Agency on Aging, Wednesday, January 16, 2008 at 9:00am for Adult Day Care and Home Care and 1:30 pm for all other services. Proposers who do not attend the proposers’ conferences do so at their own risk.
Sealed Proposals, including one (1) original bound and five (5) bound copies, must be physically in the possession of the Area Agency on Aging by 2:00pm, MST, Monday, February 25, 2008. Late proposals will not be accepted. Sealed proposals will be opened on February 25th at 2:15pm in the Area Agency on Aging conference room.
